Integrated care is understood as the systematic coordination of physical health and behavioral healthcare. Integrating mental health, substance abuse and primary care services has been shown to be the most effective approach to caring for people with multiple health needs. Integrated care is a model that works for people who can communicate and coordinate with others in order to achieve optimal patient care.  Working with, and learning from other professionals is at the heart of interprofessional collaboration and it is an area of education and practice that social workers workers must be addressing.   

 

UNC-PrimeCare, the name of a recently funded HRSA projected awarded to the School of Social Work at The UNC-Chapel Hill School of Social Work and Carolina Seminars 2015/16 are sponsoring an all-day workshop on the role of interprofessional practice and education. The morning session will offer an overview for the national imperative to move towards interprofessional education and practice. The North Carolina health care context will be addressed in addition to practical tools for practitioners and students alike. The afternoon session will address how social workers can be prepared for new models of healthcare delivery. Specifically, participants who attend the afternoon session will be able to:

 

  1. Describe a social work perspective on interprofessional collaboration: history, challenges, emerging models and opportunities
  2. Describe types of teams and implications for social work role and practice
  3. Use an Eco-map of a client situation as a tool for IP communication and collaboration
  4. Implement a simple data collection plan that will help document social work contributions to interprofessional team collaboration
  5. Discuss social work skills as applied to IP team collaboration and leadership
