Thank you for applying for the NASW-NC Myrna Miller Wellons Advocacy Award! Applications are due by December 15.

***Applications MUST consist of these FIVE items:

Completion of below form.
History of employment and/or volunteer experience (with dates) OR resume. (Either is acceptable.)
Document discussing how the nominee has advocated for policy impacting the social work profession and the clients served by social workers. Specific examples are required.
Document discussing the nominee’s involvement in professional, civic and community organizations. Give specific examples.
Two (2) letters of recommendation/support. (Letters of support may not be from nominator.)

Nominees for the award must be a North Carolina resident.
Nominees must be in good standing with the NASW Code of Ethics.
