NCCARE360 Community Organizations and Network Support Agencies Health Equity Grants

The Foundation for Health Leadership and Innovation (FHLI), the administrative organization for NCCARE360, announced the NCCARE360 Community Organizations and Network Support Agencies Health Equity Grants funding opportunity. The grants seek to support the growth and network development health of NCCARE360, the first statewide coordinated care network that connects individuals to local services and resources. Through the awarded grants, FHLI will strengthen the NCCARE360 network of community organizations that respond to social needs and provide community services. Applicants must deliver services in one of the following counties: Chatham, Durham, Franklin, Granville, Harnett, Johnston, Lee, Orange, Vance, or Wake, and can apply for the following two grant opportunities.

Organizations must submit an electronic copy of their application in PDF format to RFP@foundationhli.org by 1:00 PM on Monday, February 28, 2022. Submissions will be confirmed by reply email. Late proposals will not be evaluated. Awardees will be announced on March 7, 2022.
