NC DHHS is creating an External Partner Mind Map for the purpose of understanding the existing community organizations (and their capabilities) involved in supporting and promoting access to Medicaid Expansion across all 100 counties of NC. The intent is to identify types of organizations, the populations they serve, how they reach those populations, and where they are located. This network can then be harnessed to share time-sensitive information and content! In addition to Medicaid Expansion, NC DHHS would like to carry this map forward to inform future efforts and public health responses to ensure their critical partners are always “the first to know”. Their goal is to make the presence of this network available across all the divisions of DHHS for shared communications in the future.
Please complete this short questionnaire and provide the details of your organization. If, for some reason, you would like the department to be aware of your organization but do not want your information shared in the Mind Map, please indicate that in the questionnaire. If you have any subgroups within your organization that serve different populations or with a different reach you can add those in the last question, after which you will be prompted to answer the questions specific to your subgroup. You can do this multiple times if you have multiple subgroups. Alternatively, if you do not have subgroups, you can then submit the questionnaire.
